Building Pathways for Change

El Sol puts its mission into practice through a range of strategies and solutions that strengthen communities, elevate the CHW/Promotor workforce, and create lasting systems change. From building safe community spaces to advancing policy, developing digital tools, and providing workforce training, these efforts demonstrate the many ways we bring the CHW/Promotor model to life.

El Sol Park and Learning Center

El Sol Park & Learning Center is a promise to the community that raised us. This project was born from and shaped by parents, youth, elders, and partners who called for a safe place to gather, learn, heal, and thrive. Rooted in real stories and local priorities, it will offer wellness programs, youth leadership, outdoor education, family support services, and cultural spaces created by and for San Bernardino residents. Built through collaboration with neighbors, nonprofits, foundations, and educators, the center is designed to grow with the community, strengthen connection and trust, and expand opportunity for generations.

CHW Learning Hub

CHW Learning Hub is a growing library of resources created to support Community Health Workers and CHW/P program leaders. CHW/Ps can access free tools to build a digital profile, track training and experience, and highlight the skills they bring to their communities. Through the El Sol Training Center, organizations can access additional features that support project management, workforce development, and networking, including specialized training. Grounded in the experiences and strengths of CHW/Ps, the Hub helps strengthen connections, recognize CHW/P expertise, and support community-driven care.

LA CHW/P/R Consortium

The LA County Community Health Workers, Promotores, and Representatives (CHW/P/R) Consortium was established in 2024 to address systemic health disparities across Los Angeles County. Building on the Alliance for Health Integration Assessment and the One-Year Action Plan for the CHW Workforce, the Consortium brings together community-based organizations, CHW/Ps, and public health leaders to strengthen community-driven care. Through collaboration across health, education, and technology, we advance CHW/P training and data-informed solutions that support sustainable programs and greater equity countywide.

CommHealth Tracker

CommHealth Tracker (CHT) is a multi-agency, AI-enabled platform built to support Community Health Worker–led, value-based population health models. Designed in response to real operational challenges faced by community-based organizations, it streamlines cross-agency coordination, household-level tracking, and claims-aligned documentation while reducing administrative burden. More than a software tool, CHT functions as scalable civic infrastructure that strengthens community organizations and advances Whole Person Health.

El Sol Print Shop

Community Printing. Community Investment. Central City Prints is owned and operated by El Sol Neighborhood Educational Center. The print shop exists to support El Sol programs while reinvesting resources back into the community. Every project strengthens local capacity, creates workforce opportunities, and keeps dollars circulating within the neighborhoods El Sol serves. 

The shop produces high-quality print materials for outreach, education, and community engagement. It supports Community Health Workers, youth programs, and local initiatives with culturally responsive design and reliable production. Central City Prints reflects El Sol’s commitment to self-sustainability, community ownership, and mission-aligned operations.
